Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care across Bergen County, NJ, including Teaneck, Bogota and Paramus. All levels of experience are encouraged to apply and will train the right new graduate.

You’ll deliver 1:1 treatment, develop individualized plans, educate caregivers, and maintain accurate documentation while traveling to patients’ homes in your geographic area.

Qualifications

• Licensed Physical Therapist in NJ or eligible.
• Home health experience preferred.
• Willing to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

Education

Qualifications

• - Physical Therapist Program Graduate
• - Licensed Physical Therapist in the state of New Jersey or eligible.
• - PreferredCertifications: First Aid, AED, & CPR Certifications.
• - Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• - Ability to travel to patients’ homes within the assigned geographic territory
