Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Physical Therapist to provide in-home care across Long Island. The role offers a full-time position with a competitive salary and comprehensive benefits.

Licensed NY PT or eligible is required; travel to patients' homes within geographic territory; mentorship available for new graduates.

You will evaluate needs, develop individualized plans, document visits, and collaborate with physicians and families to support recovery and independence.

Qualifications

• Licensed Physical Therapist in NY or eligible.
• Willingness to travel to patients' homes within the geographic territory.
• Home health experience preferred but not required; mentor provided.

Responsibilities

• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.

  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible.
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Ability to travel to patients’ homes within the assigned geographic territory
  • The anticipated base salary range for this position is $85,000.00-$100,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
