Job summary

Metro Physical & Aquatic Therapy is seeking a motivated, full-time Physical Therapist to provide in-home care across New York. All levels of experience are welcome and will train the right new graduate in a progressive, supportive, and innovative team.

We offer professional growth, a supportive work environment, and flexible scheduling, with autonomy to see patients across seven days per week in your geographic area.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in NY.
• Home health experience preferred.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated base salary range for this position is $90,000.00-$100,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
