Job summary

Above and Beyond Monticello is seeking a compassionate Home Care - Registered Nurse (RN) to join our home health team. You will provide high-quality, patient-centered care in patients' homes, helping maintain health, independence, and dignity. Coverage area spans from Cedar Rapids to Monticello.

Qualifications

• Graduate of an approved nursing program (BSN preferred).
• Current RN license in the state of practice.
• At least 1 year of professional nursing experience (Home health experience is a plus!).
• Strong computer and technology skills.

Responsibilities

• Conduct comprehensive assessments of patients’ health status and needs.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Administer medications and treatments, monitor patient responses.
• Educate patients and families on disease management and health maintenance.
• Monitor vital signs and manage chronic conditions.
• Accurately document patient care activities.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Provide emotional support and counseling to patients and families.

What We’re Looking For

• Graduate of an approved nursing program (BSN preferred).
• Current RN license in the state of practice.
• At least 1 year of professional nursing experience (Home health experience is a plus!).
• Strong computer and technology skills.
• A valid driver’s license and reliable transportation.
