Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Per Diem Physical Therapist to provide in-home care in Queens. All levels of experience are encouraged to apply and will train the right new graduate.

You will evaluate patients, develop treatment plans, and deliver one-on-one therapy with a focus on restoring function, independence, and quality of life. Flexible scheduling and a supportive team culture are offered.

Qualifications

• Physical Therapist Program Graduate.
• Active New York license or eligibility to obtain one.
• First Aid, AED and CPR certifications required.
• Home health experience preferred; training available.
• Ability to travel to patients' homes within territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families and interdisciplinary team, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory

Additional Information

The anticipated pay range for this position is $70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
