Hillendale Home Care, Inc. is seeking a dynamic Home Care Community Partner to join our Santa Rosa team. You’ll develop referral relationships, coordinate with Intake and Client Care Managers, and represent Hillendale in the community.

This role focuses on building hospital and physician partnerships, guiding families through options, and ensuring a smooth transition into our care. A driver’s license and CRM experience are required.

Qualifications

• Three or more years in business development or healthcare sales.
• Ability to develop professional relationships that drive growth.
• Excellent communication and interpersonal skills.
• Strong organization and follow-through; independent worker.
• Experience with CRM software and Google Workspace; valid CA driver’s license.

Responsabilités

• Develop and maintain referral relationships with hospitals, senior care providers and physicians.
• Coordinate referrals, explain Hillendale’s services, and guide families through next steps.
• Engage the community with Lunch & Learn events and partnerships with senior living communities.
• Maintain ongoing communication with referral partners and ensure seamless admissions.
