Job summary

Homecareconcierge seeks a proactive Home Care Coordinator to manage patient transitions from facilities to our home health services. You will drive agency census growth through relationship-building and community events, host weekly facility activities, and guide families through discharge with clear communication.

No hands-on clinical care is required. You will travel daily between facilities to meet with case managers, social workers, and families, coordinating onboarding with our intake team

Qualifications

• Experience in health care settings is preferred.
• Strong relationship-building and public speaking skills are required.
• Experience planning events and activities is desirable.

Responsibilities

• Manage facility relations and outreach to drive referrals and admissions targets.
• Act as a sales partner to hit monthly and quarterly census goals.
• Host weekly facilities activities and educational seminars to boost brand visibility.
• Coordinate patient intake, ensuring smooth transitions to home care and scheduling needs.
