Qualifications

• Graduated from an accredited Occupational Therapist program.
• Must be a licensed Occupational Therapist in New York or eligible.
• Preferred experience in home health, but a mentor will assist.

Responsibilities

• Accept and treat patients in the designated geographic area.
• Evaluate and develop individualized treatment plans.
• Document patient visits and outcomes.
