Job summary

Homebridge, Inc. is seeking a Support and Retention Coordinator to assess and train Home Care Providers during the first 120 days of employment, including hands-on demonstrations in clients’ homes and classroom sessions through the Homebridge Training Program.

The role requires a strong background in home care, certified training, and the ability to manage scheduling, data tracking, and timely feedback to improve HCP performance. Travel within San Francisco is expected.

Qualifications

• Requires High School diploma or equivalent and completion of CNA/HH Aide/LVN or related home care training.
• Two years’ experience in home care or comparable work with frail elderly or disabled individuals.
• Experience working with diverse populations and cultural/linguistic backgrounds.

Responsibilities

• Assess HCPs during probationary period with visits to clients' homes.
• Provide feedback and coaching to HCPs on performance and compliance.
• Lead biweekly trainings and help develop training curricula.
• Document visits and maintain data tracking for hires and coaching notes.
