Homebridge, Inc. in San Francisco is seeking a Home Care Provider II to deliver compassionate home care for the elderly and people with disabilities. This role requires adherence to care plans and professional standards while engaging with clients who may face unique challenges.
The ideal candidate must demonstrate interpersonal skills and critical thinking while providing services that enable clients to live independently. A strong commitment to teamwork and client communication is essential.
As a Home Care Provider II (HCP II), you carry out the mission of Homebridge to provide high‑quality home care to the elderly and people with disabilities, enabling them to live safely and independently in their homes and community.
To be eligible for the HCP II position, you must be currently employed with Homebridge and have worked with Homebridge for at least four months. You must be in good standing, including showing up to work consistently and on time and maintaining professional relationships with clients and colleagues. You are not eligible if you have been in the written stage of progressive discipline in the last year.
At Homebridge, we work with a high‑risk, high‑needs population of individuals who may struggle with mental health conditions or substance abuse, live in SROs or shelters, and/or do not have a strong personal support system. As an HCP II, you are required to adhere to professional standards and Homebridge policies and procedures. You will provide services to clients who require more intensive care and oversight, implement client care plans, and report directly to your Home Care Provider Supervisor.
