Premier Health seeks an Occupational Therapist to evaluate and provide or supervise OT services in patients' homes. In the Home Care Case Manager role, you coordinate all home care components and ensure physician orders and care plans are followed.

You will document care activities and communicate with the health care team, reporting to a supervisor. Ohio licensure and CPR are required, with 1–3 years of relevant experience preferred.

Formación

• Bachelor's in Occupational Therapy from ACOTE-accredited program.
• Current Ohio licensed OT with CPR certification.
• 1–3 years of clinical experience, home health preferred.

Responsabilidades

• Coordinate home care components as Home Care Case Manager.
