Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Occupational Therapist to provide in-home care in Eastern Suffolk County. Per diem / 1099 or W-2 options; flexible schedule; travel within designated geographic area. Must be NY licensed or eligible; OT program graduate; First Aid/AED/CPR certifications.

Evening and weekend availability preferred; mentorship available for new graduates. You will evaluate, develop personalized treatment plans, educate caregivers, and document visits to

Qualifications

• Licensed OT in NY or eligible.
• OT Program Graduate.
• Willing to travel to patients’ homes.
• Strong communication skills.
• Home health experience preferred but not required.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated pay range for this position is$65-85 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
