Job summary

Metro Physical & Aquatic Therapy is seeking a Full-Time Occupational Therapist to join our Homecare team serving Riverhead and surrounding towns. The role focuses on delivering one-to-one home-based therapy, evaluating patients, and developing plans of care in accordance with physician orders.

You must be a NY-licensed OT or eligible, with willingness to travel to patients’ homes within the geographic territory.

Qualifications

• Licensed Occupational Therapist in New York or eligible.
• Graduate of an Occupational Therapist program.
• Home health experience preferred but not required.
• Ability to travel to patients' homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment to patients in their homes.
• Evaluate patients and create individualized plans of care per physician orders.
• Deliver care aligned with professional standards and agency policies.
• Educate family members and home health aides on therapy techniques.
• Plan and execute OT programs to maintain or improve function and reduce dysfunction.
• Perform initial exams, identify problems, and determine diagnoses prior to intervention.
• Provide timely documentation for each visit.
• Evaluate treatment effects over time and adjust as needed.
• Willingness to treat geriatric patients.

What You’ll Do:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated base salary range for this position is $85,000.00-$95,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
