Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Occupational Therapist to provide in-home care on the Upper East Side. All levels of experience are welcome, with mentorship available for new graduates.

Qualifications

• Licensed Occupational Therapist in NY or eligible.
• Home health experience is preferred but not required—mentoring available for documentation and compliance.
• Ability to travel to patients’ homes within geographic area.
• Strong communication skills between patients, caregivers, and management.

Responsibilities

• Accept and treat patients within designated geographic area and provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care aligning with professional standards and agency policies.
• Educate family members and caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain or restore function.
• Perform initial exam and document diagnosis prior to intervention.
• Provide timely documentation for each visit.
• Evaluate treatment effects and adjust plans for maximum benefit.
• Willingness to treat geriatric patients.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Additional Information

The anticipated pay range for this position is $75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
