Metro Physical & Aquatic Therapy is seeking a motivated Occupational Therapist to provide in-home care in Eastern Suffolk County on a per-visit basis. The position offers 1099 and W-2 options, flexibility, and opportunities for growth within a supportive team of clinicians.

The role supports treatment of geriatric patients with individualized plans and requires NY OT licensure or eligibility, plus First Aid/ CPR certifications. Training/mentoring is provided as needed.

Formación

• OT Program Graduate
• Licensed OT in NY or eligible
• Mandatory First Aid, AED & CPR Certifications
• Home health experience preferred but not required — mentor provided for documentation/compliance

Responsabilidades

• Accept and treat patients within designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders
• Deliver care aligned with standards, policies and plan of care
• Educate family members and caregivers on therapy techniques
• Plan and carry out OT programs to maintain/improve/restore function
• Perform initial exam and determine diagnosis before intervention
• Provide timely and accurate documentation for each visit
• Evaluate treatment effects and adjust as needed
• Willingness to treat geriatric patients

• Compensation: USD 65 - USD 85 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.
• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated pay range for this position is$65-85 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
