Apex Rehabilitation is seeking a Home Care Occupational Therapist (OTR) to support clients in Yonkers, NY, helping them improve independence in daily activities through personalized home-based therapy plans.

We welcome both full-time and part-time or per diem applicants who value compassionate, patient-centered care, strong clinical and communication skills, and reliable transportation for home visits.

Formación

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate and patient-centered approach to care.
• Valid Driver's License and/or reliable transportation.
• Valid Liability Insurance.

Responsabilidades

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized occupational therapy plans to address clients' goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals for holistic care.
• Educate clients and families on adaptive techniques and equipment for increased independence.
• Maintain accurate and detailed documentation of assessments, interventions, and progress.
