Apex Rehabilitation is seeking a Home Care Occupational Therapist (OTR) to support patients in their homes across Long Island and Queens. You will assess daily living needs, design personalized therapy plans, and help clients regain independence.

This role offers flexible scheduling—part time, full time, or per diem—with in-home visits, collaboration with families and healthcare professionals, and documentation to track progress. New graduates are welcome to apply and join a compassionate team.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• State licensure as an Occupational Therapist.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate and patient-centered approach to care.
• Valid Driver's License and/or reliable transportation.
• Valid Liability Insurance

Responsabilités

• Develop and implement customized occupational therapy plans to address clients' specific goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
