Job summary

Apex Rehabilitation is seeking a Home Care Occupational Therapist (OTR) to deliver in-home occupational therapy services across Sullivan County, Monticello and surrounding areas. You will assess daily living needs and create personalized plans to improve independence and quality of life for clients in their homes.

Strong clinical and communication skills are essential, along with an active OT license and a valid driver’s license.

Qualifications

• Degree in Occupational Therapy and state licensure.
• Home health care experience preferred.
• Strong interpersonal and documentation skills.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs.
• Develop and implement personalized OT plans addressing goals.
• Provide interventions to improve daily activities.
• Collaborate with clients, families, and healthcare professionals for holistic care.
• Educate on adaptive techniques and equipment for independence.
• Maintain accurate documentation of assessments and progress.

Key Responsibilities:

Qualifications:

• Valid Driver's License and/or reliable transportation.
