Job summary

ComForCare Home Health Care (North San Diego) is looking for compassionate Home Care Nurses, LVNs and LPNs to join our growing team in San Diego, CA. You will provide one-on-one in-home nursing care, assess patients and implement care plans, and educate families to support health and safety.

Qualifications

• Active RN or LVN license in California.
• Experience in home health or community nursing preferred.
• Strong clinical assessment and critical thinking.
• Excellent communication and patient education abilities.
• Valid California driver's license and reliable transportation.
• CPR/BLS certification required.
• Compassionate, self-motivated, and able to work independently.

Responsibilities

• Conduct in-home patient assessments and develop care plans.
• Administer medications, treatments, and nursing procedures as prescribed.
• Monitor and document patient health status and changes.
• Coordinate with physicians and care team for continuity of care.
• Educate patients and families on health management and safety.
• Provide wound care and IV therapy as needed.
• Maintain accurate clinical documentation in compliance with regulations.

Requirements:

• Active RN or LVN license in the state of California
• Previous home health, home care, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Valid California driver's license and reliable personal transportation
• CPR/BLS certification required
• Compassionate, self-motivated, and comfortable working independently
