Job summary

ComForCare Home Health Care (North San Diego) is seeking a compassionate Home Care Nurse to provide in-home nursing services in the San Diego area. You will deliver patient-centered care, coordinate with the care team, and support families in managing health needs.

The role requires California RN or LVN licensure, strong clinical assessment, effective communication, and the ability to work independently in patients' homes. Training and flexible scheduling are offered to support caregivers.

Qualifications

• Active RN or LVN license in California.
• Home health or community nursing experience preferred.
• CPR/BLS certification required.

Responsibilities

• Conduct in-home patient assessments and develop individualized care plans
• Administer medications, treatments, and skilled nursing procedures as prescribed
• Monitor and document patient health status, progress, and changes in condition
• Coordinate with physicians, therapists, and care team members to ensure continuity of care
• Educate patients and family members on health management, medications, and safety
• Provide wound care, IV therapy, and other skilled nursing interventions as needed
• Maintain accurate and timely clinical documentation in compliance with regulations
