ComForCare Home Health Care (North San Diego) is seeking a compassionate Licensed Vocational Nurse (LVN) to join our home health team in San Diego, CA. You will deliver one-on-one care in patients' homes, including wound care and medication administration, while developing individualized care plans.

We value patient-centered care, flexible scheduling, and a supportive team. Active California LVN license and CPR/BLS certification are required, with prior home health or clinical care experience

Qualifications

• Active California LVN license
• Current CPR/BLS certification
• Home health or clinical care experience preferred
• Knowledge of wound care and medication management
• Strong communication and organizational skills

Responsibilities

• Provide direct patient care in home settings, including wound care and vital sign monitoring
• Develop and follow individualized patient care plans with supervising RN
• Observe, document, and report changes in patient condition to clinical staff
• Administer prescribed medications and treatments per physician orders
• Educate patients and families on care procedures and health management
• Maintain accurate clinical documentation in compliance with state and agency standards
• Coordinate with care teams to ensure continuity of patient care
