Tanglewood Manor/Memory Garden in Jamestown, NY is seeking a Home Care Nurse (RN) to conduct in-home assessments, develop and update care plans, manage medications, and coordinate with physicians, families, and Medicaid providers from a flexible hybrid work setting.

Qualifications

• Current New York State RN license (active)
• Current CPR/BLS certification
• Graduate of an accredited School of Nursing (ADN required; BSN preferred)
• 1+ years of nursing experience; home care/geriatrics valued
• Key skills: self-direction, time-management, clinical assessment, empathetic communication, and remote collaboration tools
• Valid driver’s license and reliable vehicle, willing to travel locally

Responsibilities

• Conduct in-home assessments, evaluations, and safety appraisals during self-scheduled client visits
• Design, update, and maintain Care Plans from home or local office, collaborating virtually with physicians, families, and Medicaid providers
• Oversee medication management, track vitals/symptoms, advise on safe routines via in-person visits and remote check-ins
• Perform regular visits and follow-ups to evaluate health changes and care goal attainment
• Educate clients and family caregivers on self-care, disease management, fall prevention, and wellness strategies
• Coordinate with community resources, transportation, dietary supports, and day programs
• Provide oversight to PCAs and HHAs through in-person checks and virtual support

Responsibilities

• Conduct flexible in-home assessments, evaluations, and safety appraisals during self-scheduled client visits

• Design, update, and maintain comprehensive Care Plans from your home or local office, collaborating virtually with primary care physicians, families, and Medicaid Managed Care Providers

• Oversee medication management, track client vitals/symptoms, and advise on safe routines via in-person visits and remote check-ins

• Conduct regular visits and phone/video follow-ups to evaluate health changes, monitor safety, and ensure care goals are met

• Educate clients and family caregivers on self-care, disease management, fall prevention, and wellness strategies

• Serve as the central clinical contact, coordinating with community resources, transportation, dietary supports, and day care programs

• Provide oversight and guidance to personal care aides (PCAs) or home health aides (HHAs) through periodic in-person checks and virtual support

Requirements

• Active, unencumbered Registered Nurse (RN) license in the State of New York

• Current CPR/BLS certification required

• Graduate of an accredited School of Nursing (ADN required; BSN preferred)

• 1+ years of nursing experience; prior experience in home care, care management, community health, or geriatrics is highly valued

• Key Skills: Self-direction, excellent time-management, clinical assessment expertise, empathetic communication, and comfort with digital/EMR systems and remote collaboration tools
