Be the Nurse Who Sets the Standard of Care. Join a Team That Puts PEOPLE First.
People Care has served New York patients and families for 50 years and we are looking for you to enter the next 50.
At People Care, we believe exceptional care and service depends on exceptional staff. Since 1976, we've been helping New Yorkers live safely and independently at home while providing our caregivers with the support, training, flexibility, and respect they deserve. Employing roughly 4,300 caregivers across New York City and Long Island, we're seeking a skilled, compassionate registered nurse who will see patients at home, set the standard of care, and hold our aide team accountable to it.
About the Position
The Field Nurse is a registered professional nurse who visits patients at home to assess them, develop and maintain the plan of care, and supervise home health aides and personal care aides in accordance with 10 NYCRR Part 766. The role ensures care in the home is safe, appropriate, ordered, and fully documented. When field nursing works well, patients are safe, care plans are current, aides know what to do, and the agency stays in compliance. Every patient relationship starts with this visit.
RESPONSIBILITIES
- Conduct the start-of-care (SOC) comprehensive assessment in the home: physical, cognitive, functional, psychosocial, nutritional, medication, and home safety
- Develop an individualized plan of care consistent with physician/NP orders, specifying aide tasks, frequency, and duration
- Reassess and update the plan of care with any change in condition, after hospitalization (resumption of care), and at least every six months
- Verify current medical orders are in place for all services; obtain or clarify orders as needed
- Assign each patient's emergency preparedness priority level and individual emergency plan
- Review Patient Rights and Responsibilities, the complaint/grievance process, and required agency notices with the patient or representative at SOC, and document the review
- Educate patients and families on the plan of care, safety, infection control, and when to contact the agency or provider
- Orient the aide to each patient's plan of care in the home at the start of service
- Conduct on-site supervisory visits at the frequency required by regulation and agency policy, evaluating aide competency, adherence to the plan of care, and patient satisfaction
- Identify performance or training needs and refer aides for in-service or remediation
- Report aide concerns, incidents, or suspected abuse, neglect, or mistreatment immediately per agency policy and NYS requirements
- Communicate changes in condition, falls, hospitalizations, and safety concerns promptly to the provider, Director of Patient Services, and care team (including MLTC care managers)
- Participate in discharge planning and prevention of avoidable hospitalizations
- Collaborate with the Nurse Educator and Compliance on complaints, incidents, and corrective actions
- Complete and submit accurate visit documentation within agency timeframes, using the SOC packet checklist and approved tools
- Attend scheduled in-office documentation sessions
- Accept visit assignments through administrative scheduling; nurses do not self-schedule SOC visits
- Follow EVV, infection control, HIPAA, and PHI-protection policies
