FreedomCare is seeking a Licensed Social Worker (LSW) in Ohio for a full-time, hybrid role requiring fieldwork and bi-monthly travel to the Cleveland office. The LSW will oversee the Structured Family Caregiving program, supporting patients and caregivers while ensuring compliance with HIPAA and Ohio regulations.

The ideal candidate holds an Ohio LSW license, has 2+ years of clinical experience (elderly/disability experience a plus), and can leverage Salesforce CRM.

Qualifications

• Active Ohio Licensed Social Worker license (CSWMFT Board).
• Bachelor’s or Master’s in Social Work from CSWE-accredited program.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities.

Responsibilities

• Interpret the agency philosophy to clients and families.
• Ensure compliance with HIPAA and state regulations.
• Conduct initial in-person home assessments with patient and caregiver.
• Provide monthly contact regarding enrollment, credentialing, and caregiver support.
• Maintain detailed case records following agency policy and procedures.
• Escalate and contribute to the patient’s plan of care and physician plan of treatment.
