Job summary

Entyre Care is seeking a detail-oriented Care Manager to conduct home visits, train caregivers, and support ongoing client care. You will monitor client health, help create personalized care plans, and ensure compliance with regulatory standards. This role emphasizes compassion, quality care, and caregiver empowerment.

Qualifications

Bachelor's degree required; LSW or LISW
At least one year of experience with elders or adults with disabilities
Valid driver's license and automobile insurance; travel within Ohio
Experience in case management, care planning, and assessment in healthcare
Proficiency in Microsoft Office applications (Excel/Word/PowerPoint/Teams)
MSW or related field preferred
SFC experience or similar setting preferred
Gerontology or case management certifications preferred

Responsibilities

Home visits to assess and create personalized care plans for clients and caregivers
Train and support caregivers, ensuring they can meet loved ones' needs
Monitor and report on clients’ health, coordinating with primary care physicians and community resources
Facilitate smooth transitions and ongoing care for clients
Maintain compliance with relevant regulations and standards
Participate in community events to spread awareness and enroll new caregivers
