Job summary

Entyre Care is seeking a detail-oriented Care Manager to conduct home visits, train and support caregivers, and ensure high-quality, compliant care for clients at home.

You will monitor clients' health, develop personalized care plans, coordinate with primary care physicians and community resources, and advocate for excellent caregiver support. Requires LSW/LISW, Ohio travel, and proficiency with standard office software.

Qualifications

• Bachelor's degree required; must hold LSW or LISW.
• At least one year of experience with elders or disabled adults.
• Driver's license and auto insurance; travel within Ohio.
• Experience with case management, care planning, and health assessments.

Responsibilities

• Conduct home visits to assess and create personalized care plans.
• Train and support caregivers; ensure they can meet loved ones' needs.
• Monitor clients’ health and report to primary care physicians.
• Coordinate with community resources and ensure ongoing care.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with relevant regulations and standards.
