Job summary

Entyre Care is seeking a detail-oriented Care Manager to perform home visits, train and support caregivers, and develop personalized care plans for clients and their families in Ohio. You will monitor health, ensure ongoing quality care, and maintain regulatory compliance while coordinating with primary care physicians and community resources to deliver compassionate, high-quality home care.

Qualifications

• Bachelor's degree required; LSW or LISW preferred.
• At least one year of experience with elders or adults with disabilities.
• Valid driver's license and auto insurance; travel within Ohio.
• Experience in case management and care planning in healthcare.
• Proficient in MS Office applications (Excel, Word, PowerPoint, Teams).

Responsibilities

• Conduct home visits and create personalized care plans.
• Train and support caregivers to meet loved ones' needs.
• Monitor clients' health and report to primary care physicians.
• Coordinate with community resources for ongoing care.
• Ensure compliance with regulations and standards.
• Participate in community events to enroll new caregivers.
