Job summary

Hartford HealthCare at Home is seeking a licensed practical nurse to provide intermittent skilled nursing care to clients in home and community settings under the supervision of an RN. You will follow the plan of care and support patient outcomes in the home.

CT LPN licensure, valid CT driver's license, and EMR proficiency are required. You will record daily progress, communicate with the primary nurse and other disciplines, and participate in care planning with a multidisciplinary team.

Qualifications

• CT state approved LPN certificate program completed.
• CT LPN license currently valid.
• Valid CT driver's license and clean driving history.
• Strong computer skills including Outlook, Word, and Excel.

Responsibilities

• Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.
• Records daily progress in EMR.
• Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.
• Communicates/conferences With Primary Nurse And Other Disciplines.
• Consults with physician regarding client needs and plan of care.
• Assists in evaluation of care given by paraprofessional staff.
• Participates on agency committees and special projects.
