Rockaway Home Care in New York seeks a Home Care Intake Specialist to join our Inwood office. This is an in-office administrative role focused on referral processing, enrollment, insurance verification, authorizations, and follow-up; no hands-on care required.

You'll serve as a key contact for referrals, verify eligibility, coordinate with Medicaid/MLTC, and ensure documentation is complete. Applicants should have at least one year of intake experience and strong organizational skills.

Home Care Intake Specialist – Michigan

In-Office Administrative Position

This is an in-office administrative position, not a caregiver or CNA role. No CNA license is required. You will not provide hands-on care. This role focuses on referral processing, consumer enrollment, insurance verification, authorizations, and administrative follow-up.

Rockaway Home Care, a division of Foundation For the Elderly, is seeking a dependable and detail-oriented Home Care Intake Specialist to join our team serving consumers throughout Michigan based in our Inwood, NY office.

This position is ideal for someone with experience in home care intake, referrals, insurance verification, Medicaid, or administrative coordination who is organized, comfortable managing multiple cases, and committed to providing excellent customer service.

What You'll Do

As a Home Care Intake Specialist, you will serve as a key point of contact for new referrals, helping consumers and their families navigate the home care enrollment process while ensuring documentation, eligibility requirements, and authorizations are handled accurately and efficiently.

Responsibilities include:

• Receive, review, and process home care referrals from healthcare providers, facilities, community organizations, and other referral sources.

• Contact consumers and families to explain available services and collect required information.

• Enter, review, and verify consumer information in internal systems.

• Verify insurance coverage and eligibility for home care services.

• Review referral information and identify missing documentation or additional requirements.

• Coordinate with Medicaid programs, MLTC plans, insurance providers, and community resources, as applicable.

• Submit referrals to appropriate plans or resources when enrollment or additional assistance is needed.

• Follow up on insurance approvals and service authorizations.

• Confirm which services have been approved and ensure required authorizations are obtained before services begin.

• Communicate case updates and relevant information to the Coordination Department.

• Maintain accurate records and timely documentation in internal systems.

• Follow up consistently on pending referrals, authorizations, and outstanding paperwork.

• Communicate professionally with consumers, families, referral partners, insurance representatives, and internal teams.

• Help ensure a smooth and timely enrollment process for each consumer.

What We're Looking For

• Minimum of one year of home care intake experience required.

• Knowledge of home care intake, referral processing, and enrollment procedures.

• Understanding of insurance verification, Medicaid, and MLTC processes.

• Experience working with Medicare, Medicaid, insurance providers, or managed care plans preferred.

• Strong verbal and written communication skills.

• Excellent organizational skills and attention to detail.

• Comfortable with data entry, Microsoft Office, and learning new computer systems.

• Strong follow-up skills and the ability to manage multiple cases simultaneously.

• Ability to investigate information, identify missing documentation, and determine appropriate next steps.

• Self-motivated and able to work independently with minimal supervision.

• Professional, dependable, and team-oriented.

• Strong customer service skills when communicating with consumers, families, and referral partners.

• Ability to work full-time in person at our Inwood office.
