Job summary

The Field Supervisor is responsible for conducting on-site client evaluations and developing service plans in conjunction with the authorization and performs other field visits as assigned. They complete visits as scheduled, report daily to the scheduler, recruit, hire, train, and supervise attendants, and ensure all forms are completed and submitted within 48 hours.

Qualifications

• Two years college study or HS diploma with supervisory experience acceptable.
• PHC, MC experience preferred.

Responsibilities

• Complete visits as scheduled and report daily to scheduler.
• Recruit, hire, train, and supervise attendants.
• Complete forms and attach to daily visit report; submit within 48 hours.
• Maintain supervisor files and confidentiality.
• Participate in staff meetings and community relations.
