Job summary

Addus is seeking a Field Supervisor to conduct on-site client evaluations and develop service plans in collaboration with authorization. The role includes visiting clients, supervising attendants, and ensuring timely reporting and paperwork submission.

Qualifications

• Two years of full-time study at an accredited college or university or individuals hired with a high school diploma or equivalent may substitute one year of full-time supervisory employment in a healthcare facility or agency for each required year of college.
• PHC, MC experience preferred.

Responsibilities

• Completes visits as scheduled and reports daily to scheduler.
• Recruits, hires, trains, and supervises attendants.
• Completes forms attached to the daily visit report and submits to the office within 48 hours of visit.
