Job summary

Granite VNA, Inc. is seeking a Registered Dietitian to support Home Care and Hospice patients in the Concord, NH area. This per diem position involves flexible, approximately 5 hours per month, with direct patient care and collaboration with an interdisciplinary team.

You will assess nutrition, educate patients and caregivers, and document outcomes in the EMR while providing specialized nutrition care in a community setting.

Qualifications

• Bachelor of Science in Nutrition or related field.
• Post-graduate internship completed.
• Registered by the Commission on Dietetic Registration.
• Licensed by the State of New Hampshire.
• Minimum of five years of clinical experience.

Responsibilities

• Perform nutrition assessments and education for referred patients via telephone, video, or home visit.
• Develop individualized nutrition care plans based on clinical condition and home environment.
• Communicate nutrition recommendations to case managers and medical providers.
• Document assessments, interventions, and follow-up plans in the EMR.
• Educate patients and caregivers on diets, nutrition goals, and feeding regimens as needed.
• Monitor patients on nutrition support and address related concerns.
• Provide in-service education to Home Care and Hospice staff as needed.
• Serve as nutrition resource for interdisciplinary clinical teams.
