Qualifications

• Licensed by the State of New Hampshire.
• Minimum of five years of clinical experience.

Responsibilities

• Perform nutrition assessments and education for referred Home Care and Hospice patients via telephone, video, electronic communication, or home visit, as clinically indicated.
• Develop individualized nutrition care plans based on patient condition, goals of care, and home environment.
• Communicate nutrition recommendations to case managers, medical providers, and Home Care teams.
• Document assessments, interventions, patient education, and follow-up plans in the EMR.
• Educate patients and caregivers on diets, food selection, and nutrition goals.
• Monitor nutrition support and troubleshoot related concerns.
• Educate staff on the role of the Registered Dietitian within Home Care and Hospice.
