Bluegrass Care Navigators is seeking a Home Care Certified Nursing Assistant (CNA) to deliver one-on-one personal care in participants' homes across Kentucky, under RN supervision.

The role supports independence and safety, performing daily living activities, light housekeeping, and meal prep while documenting observations for the Interdisciplinary Team (IDT). Strong communication and flexibility are essential.

Formación

• High school diploma or GED required.
• SRNA certification through Kentucky Board of Nursing required.
• Completion of an approved CNA training program required.
• Minimum 1 year of experience with frail/elderly population or training provided on hire.
• CPR certification maintained.

Responsabilidades

• Provide direct personal care and supportive services in participants' homes.
• Assist with activities of daily living, light housekeeping, and meal preparation.
• Monitor participant condition and report changes to the RN and IDT to ensure timely intervention.
• Maintain independence, safety, and quality of life for participants.
