Job summary

All Ways Caring HomeCare in West Virginia seeks a dedicated case manager to oversee follow-up with ADW services and ensure service plans are followed. You will coordinate with providers, support families, and maintain documentation.

The role requires a WV licensed social worker or counselor, a Bachelor’s in Human Service, and at least two years in case management. Regular travel may be required to meet six-month and annual plan meetings.

Qualifications

• Two years of case management experience or related discipline.
• Licensed in West Virginia as a Social Worker or Counselor.
• Proficiency with MS Word and Excel; ability to use standard software for documentation.

Responsibilities

• Follow up with persons receiving ADW services to ensure service plan delivery.
• Submit MNER according to policy and maintain eligibility records.
• Coordinate six-month and annual service plans with providers.
• Provide service plans to applicable providers and keep client informed.
• Assist with grievances, complaints, and fair hearings; refer for legal representation when needed.
• Document in WV IMS and complete required reports for BMS or OA.
