Circle of Life is hiring a Case Coordinator in Gallup, NM. This full-time role focuses on onboarding PCA staff, client assessments, scheduling, and ongoing liaison with clients and care providers.

The position emphasizes accurate record-keeping, training setup, and responsive service delivery to support independence at home. The ideal candidate will have an associate degree or equivalent and strong skills in Microsoft Office tools, time management, and compassionate client care.

Qualifications

• Associate degree or equivalent from two-year college, technical school, or related experience.
• Proficiency with Microsoft Office Suite (Outlook, SharePoint, Excel, Teams).
• Ability to manage time, handle stressful situations, and meet deadlines.
• Willingness to assist, encourage, and honor those we serve.

Responsibilities

• Process PCA new hire applications and assist onboarding.
• Conduct monthly home visits and client wellness visits as needed.
• Process client inquiries, assessments, and requests.
• Maintain accurate system records and employee records.
• Create schedules for Personal Care Attendants per care plan.
• Communicate with care providers, clients, and PCAs for updates.
• Report PCA and client incidents and follow up on escalated concerns.
• Supervise PCAs by setting attendance and performance expectations.
• Set up training for new or current employees.
• Perform caseload audits and respond to audit requests.
• Ensure the office workspace is professional and organized.
• Participate in weekly PCA/Client meetings with Regional Director.
• Attend local promotional and outreach events.
