Job summary

Caring Hands Homecare is seeking a Home Care Aide to provide one-on-one in-home care to patients in New Jersey. You will follow individualized care plans, support daily activities, and maintain detailed care records. Strong communication is essential, and prior home health or hospice experience is a plus.

Applicants must be a graduate of an accredited Home Health Aide program and hold NJ CHHA certification. A valid driver's license and CPR certification are preferred.

Qualifications

• Graduate of an accredited Home Health Aide program.
• High School Diploma or GED required.
• One year of professional experience preferred.
• Driver's license preferred.
• CPR certification preferred.
• New Jersey Certified Home Health Aide Certification REQUIRED.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, and mobility aids.
• Care for patients by performing housekeeping tasks and personal care.
• Maintain records of patient care, condition, progress, or problems to report to the supervisor.
• Build relationships with patients through conversation to support mental well-being.
• Administer medications and immunizations as directed.
• Engage patients in exercises or activities to promote health.

Job Summary

We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.

Responsibilities

• Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
• Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
• Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
• Give medications and immunizations
• Engage patients in exercises or other activities
