ViaQuest Psychiatric & Behavioral Solutions in Toledo, OH is hiring a Community Based/Day Program Therapist to provide comprehensive diagnostic assessments and ongoing therapy for diverse clients in home and community settings.

This full-time role includes licensure supervision, extensive training, a rich benefits package (medical, vision, dental, life, 401k), cell phone reimbursement, PTO, mileage reimbursement, flexible scheduling and an employee referral bonus program.

Qualifications

• Bachelor's degree in Social Work or related field with one year of therapy experience, or a Master's in Social Work/Professional Counseling.
• Licensed in Social Work or Counseling (LSW/LISW/LISW-S/LPC/LPCC/LPCC-S) or equivalent.
• Therapy and counseling experience preferred.
• Home-based therapy experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel within service area.

Responsibilities

• Perform comprehensive diagnostic assessments.
• Provide patient visits as scheduled.
• Ensure treatment plans are up to date for each client.
• Complete comprehensive and succinct progress notes.
• Communicate and follow up as needed regarding the client's status.
• Work collaboratively with case management and/or psychiatric services.
