Job summary

Homeside Rehab in Syracuse, NY is seeking a compassionate Speech-Language Pathologist to join our pediatric home health team. You will provide individualized, evidence-based speech therapy services to patients in their homes while enjoying schedule flexibility and a supportive team environment.

The position requires NY SLP licensure, at least one year of relevant experience, and strong communication skills.

Qualifications

• Active NY State Speech-Language Pathologist license required.
• Minimum 1 year of experience in home health, outpatient, or SNF settings preferred.
• Basic computer literacy for documentation and communication.
• Strong clinical and interpersonal communication skills.
• Ability to work independently in a home health environment.
• Reliable transportation for home visits preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on needs and goals.
• Provide evidence-based treatment for speech, language, fluency, articulation, and voice disorders.
• Maintain accurate and timely documentation of patient care.
• Collaborate with the clinical team to ensure coordinated and effective treatment.
• Communicate effectively with office staff, patients, and caregivers.
• Collaborate with healthcare professionals to ensure comprehensive care.
