Job summary

Apex Rehab Group in Westchester County, NY is seeking an experienced, licensed Speech Language Pathologist to provide in-home therapy. You will assess patients, design personalized treatment plans, document care, and adjust goals for progress.

This role offers flexible scheduling (part-time, full-time, or per diem) with competitive pay and health benefits. You will work closely with patients and families to empower successful communication.

Qualifications

• Active NY state license for practicing speech therapy.
• Strong communication and interpersonal skills.
• Excellent organizational skills.
• Strong charting and documentation skills.
• Prior experience in home care is preferred but not required.

Responsibilities

• Provide therapy services to patients in their homes.
• Make evaluations and create treatment plans with short/long-term goals.
• Assist in implementing treatment plans and adjust as needed.
• Maintain accurate records of patient needs, care and outcomes.
• Collaborate with families to ensure understanding and success.
