Job summary

Personal Touch Home Care is seeking a full-time Speech Language Pathologist (SLP) to deliver high-quality, home-based care across East New York and Canarsie in Brooklyn, NY. The role supports a pediatric and adult caseload with 30–60 minute sessions, emphasizing mobility and independence in patients' homes.

Applicants must hold a NYS SLP license and a graduate degree in Speech-Language Pathology. Excellent communication and organization skills are essential; home care experience is a plus.

Qualifications

• Active NY SLP license required.
• Graduate of an accredited SLP program.
• Strong communication and organizational skills.
• Home care experience preferred, but not required.
• Experience with pediatric, adult, special needs, or geriatric populations is a plus.

Responsibilities

• Conduct comprehensive assessments of speech and language abilities.
• Develop and implement individualized treatment plans tailored to each patient's needs.
• Provide therapy to individuals of all ages, addressing articulation, fluency, voice, and language.
• Collaborate with other healthcare professionals to provide holistic care.
• Monitor and document patient progress, adjusting treatment plans as necessary.
• Educate patients and families about communication strategies and home exercises.
• Stay current with best practices and advancements in speech therapy.

Education

Master's degree in Speech-Language Pathology

Active New York State SLP license
