Qualifications

• Must hold valid Arizona PT or OT license.
• Experience in outpatient or home-based care preferred.
• Reliable transportation for travel within Phoenix metro area.
• Strong interpersonal skills and self-motivation.

Responsibilities

• Conduct in-home evaluations and treatments for outpatient PT/OT patients.
• Create tailored treatment plans to improve mobility and recovery.
• Maintain clear documentation and patient communication.
