Job summary

AdvisaCare is seeking a dedicated Home Health Occupational Therapist to join our team on a PRN basis in Phoenix, AZ. You will work one-on-one with clients in their homes, helping them regain independence and achieve personal goals.

Responsibilities include conducting assessments, developing plans, delivering OT services, documenting progress, and educating families. A valid AZ OT license, 1 year home health experience, and a reliable vehicle are required to ensure coordinated care and

Qualifications

Active Arizona OT license in good standing.
Minimum of 1 year OT experience, preferably in home health.
Ability to work independently with a flexible schedule.
Strong communication and interpersonal skills.
Comfortable with technology for documentation.
Must have a valid driver's license and reliable transportation.

Responsibilities

Conduct patient assessments and develop individualized treatment plans
Provide occupational therapy services to patients in their home environment
Document patient progress and maintain accurate records
Educate patients and their families about home exercise programs and daily living activities
Collaborate with other healthcare professionals to ensure coordinated care
Participate in team meetings and conferences to discuss patient care plans
Communicate effectively with patients, families, and physicians
