Job summary

Health Wealth Safe, Inc. is seeking a Nurse Practitioner to provide home-based primary and palliative care for medically complex patients in a physician-led, interdisciplinary model.

You will conduct in-home and telehealth visits, assess and manage chronic and acute conditions, and work with nurses, social workers, and care managers to deliver compassionate, patient-first care. This role aims to reduce avoidable hospitalizations through proactive planning and follow-up.
