Job summary

U.S. Department of Veterans Affairs Fort Knox VA Clinic is seeking a Registered Nurse for Home-Based Primary Care Outpatient Staff. The HBPC RN delivers Veteran-centered care through home visits, coordinates case management, education, and medication/treatment plans for Veterans with complex needs.

The role emphasizes leadership, care coordination, and adherence to VA nursing standards, with a standard daytime schedule and strong emphasis on teamwork within the HBPC program.

Qualifications

• Must be proficient in basic written and spoken English
• Graduate of a nursing program approved by accrediting bodies (ACEN/CCNE)
• Bridge program candidates may become registered nurses upon licensure
• State licensure as a nurse upon appointment
• Foreign nursing education must have degree equivalency validation
• Current full, active, unrestricted nursing license in US territory or DC
• Pre-placement physical examination may be required

Responsibilities

• Provides direct Veteran care via home visits and care coordination within HBPC
• Plans and delivers technically competent care in the home and community setting
• Leads and participates in policy and procedure development and evaluation
• Provides feedback and supports professional development of HBPC staff
• Ensures safe, high-quality nursing care and optimizes Veteran outcomes
• Contributes to program-level HBPC outcomes and performance improvements
