Job summary

Qualifications

• Graduate of a school of professional nursing with an active RN license in the U.S.

Responsibilities

• Directs Veteran care and care coordination within the HBPC Program
• Plans and delivers technically competent care in the home of Veterans with complex needs
• Assesses, plans, implements and evaluates care focused on education, self-management, and satisfaction
• Administers medications and procedures per established policies and guidelines
• Utilizes interventions to maximize health and reduce urgent care visits
• Makes home visits to Veterans enrolled in HBPC
• Works with the interdisciplinary team to improve care outcomes
