Job summary

Housecall Providers Inc. seeks a Physician Assistant specialized in primary care to deliver medical care in patient residences, focusing on chronic disease management and palliative care.

Collaboration with a licensed physician and after-hours coverage are part of the role. Responsibilities include comprehensive health assessments, treatment plans, referrals, and coordination with care facilities to ensure continuity of care in community settings across Marion and Polk counties, Oregon.

Qualifications

• Master’s level education with major coursework in primary care for physician assistants or related field.
• Licensed physician assistant in the State of Oregon.
• DEA license and NCCPA certification.
• CPR certification at hire or within 6 months.

Responsibilities

• Provide diagnosis, treatment, consultation, and follow-up care with physician collaboration.
• Perform comprehensive health assessments and develop patient-centered care plans.
• Order testing and medications, and coordinate referrals and follow-up.
• Provide after-hours call coverage and maintain medical records per protocols.
