Job summary

AuthoraCare Collective is seeking a Nurse Practitioner for Home Based Primary Care in High Point, NC. This full-time role involves delivering comprehensive primary and palliative care to home-bound patients, performing exams, diagnosing illnesses, and coordinating with the care team.

Travel to patient homes within the service area and on-call coverage are required. The position requires CPR/BLS certification and a nursing degree from an accredited school, with two years of acute care or home

Qualifications

Education and Experience: Graduate of an accredited School of Nursing.
CPR/BLS certification required.
Two years of acute care, home health, long-term care, or outpatient/office experience preferred.

Responsibilities

Deliver comprehensive primary and palliative care to home-bound patients.
Perform physical examinations, diagnose illnesses, and manage chronic conditions.
Coordinate care with interdisciplinary team and other healthcare providers.
Provide health promotion, disease prevention, and patient education.
Travel to patients' homes within service area; participate in on-call coverage.
