Job summary

Therapy 2000 is seeking a Pediatric Physical Therapist to join a mission-driven team providing home-based pediatric therapy across Highland Park/University Park/East Dallas. You will perform comprehensive PT evaluations, develop individualized plans, and collaborate with families and healthcare teams to support children in reaching their goals.

Experience in pediatric home-health, bilingual English/Spanish, and 3+ years as a pediatric PT are preferred.

Qualifications

• 3+ years as a pediatric Physical Therapist
• Experience in a home-health setting
• Specialty Certifications (example: PCS)
• Bilingual (English/Spanish)

Responsibilities

• Perform comprehensive Physical Therapy evaluations using standardized tests to determine patients' needs for therapy.
• Develop and implement treatment plans aligned with physician orders.
• Create home exercise programs in partnership with patients' caregivers.
• Collaborate with other providers to coordinate care in the patient’s medical neighborhood.
• Meet weekly visit requirements and complete chart updates and documentation.
