Job summary

Choice Health is seeking a Nurse Practitioner to build a patient panel delivering home-based palliative care, focusing on symptom management and patient education. You will work with families to discuss options and collaborate with the Medical Director and care team.

The role emphasizes leadership, education, and research within a community-based setting, with travel to patients' homes and facilities as needed. Salary is production-based with bonuses.

Qualifications

• Graduate of a recognized RN program required.
• Master's degree in Nursing required.
• 1-3 years work experience as a Nurse Practitioner required.
• Current APRN license by Texas State Board of Nursing required.
• Prescriptive authority from TX TMB and Federal DEA.
• BLS certification.
• ANCC NP certification or equivalent required.
• ACHPN certification within 12 months of employment.

Responsibilities

• Provide community-based palliative care for patients focused on symptom management.
• Confer with Medical Director or supervisor regarding assigned patients.
• Keep physicians informed of needs and problems in patient care.
• Coordinate post-discharge palliative care with discharge planners.
• Participate in inter-disciplinary team meetings and related activities.
• Educate health professionals and community members about palliative care.
